PURCHASE ORDER (mail or fax): Send a school purchase order to the address below or fax it to Learning Center USA at 1-585-593-0313. All purchase orders must have a signature from an authorized purchasing agent. Payment is due 30 days after receipt of materials on orders from schools and institutions.
OUR ORDER FORM (mail or fax): Please mail completed order forms to the address below or fax them to Learning Center USA at 1-585-593-0313. If you do not have a Learning Center USA order form, contact us and request that one be sent to you via mail or fax.
Mailed order forms must include a check, money order, or credit card number (VISA, MasterCard, or DISCOVER only). If you are paying by check, we reserve the right to ship after the check clears. A certified check or money order will ensure immediate shipment of your order.
Faxed order forms must include a credit card number, expiration date, authorized signature, and telephone number.
Please note: If this order form is used by a school or institution, it must be accompanied by a purchase order and include the signature of an authorized purchasing agent.
